Job Description:

The Clinical Quality Improvement Specialist will support hospital-wide performance improvement efforts through facilitation for improvement to the stroke program. The CQI Specialist will act as an internal consultant for all levels of the organization providing team facilitation, coaching, change management, and process improvement resulting in successful stroke program metrics. The CQI Specialist will provide education and training for teams and departments on performance improvement tools and strategies with lean principles. This position will also support the stroke program in review of concerns regarding the quality of clinical care provided. Will work under the direction of the Manager of the Stroke Program to ensure assignments are completed according to time frames, certification organization requirements, and quality expectations. Assist in performing case reviews/audits for evaluation for all Stroke program metrics related to the hospital s certification as a comprehensive stroke center. Monitors stroke indicators and designs activities by working with staff, managers and directors as appropriate. Conducts daily review of stroke patient records to ensure care provided is according to hospital stroke guidelines/policies. Collects stroke data on required measures by certifying organizations Assists Manager in preparation for monthly stroke committee meetings Coaches staff daily and in a timely fashion (clinical and non-clinical staff) when specific elements (core stroke measures) are not met. Be able to analyze stroke quality data to determine areas of improvement and recommend activities to improve hospital performance in the stroke program Provides assistance to surveyors in review of clinical records and policies as appropriate and needed. Provides ongoing education and training pertinent to stroke performance improvement based on staff needs and assessments. Facilitate teamwork and communications with staff (clinical and non-clinical) to assure optimal outcomes in the provision of stroke performance improvement services to the organization. Ensures confidentiality with patient and physician information, PHI, and demonstrates understanding of HIPAA. Assists the Manager of the stroke program with educational efforts that may extend to the community. Develops positive relationships with staff and identifies areas that require quality improvements in the stroke program Assists directors & managers with design of monitoring programs to meet performance improvement plans. Answers the phones and responds to patient and or customer needs professionally. Works effectively and efficiently within the department. Willingly takes on additional assignments as needed Reports to work on scheduled on time, sufficiently rested to perform duties of the job. Takes initiative in keeping the Manager fully informed on work related issues. Attends and take minutes for stroke committee meetings and meetings associated with stroke work Maintains files and documentation as required by certification organizations Qualifications: Required: Current CA RN license with a minimum of 3 to 5 years of work experience in an acute hospital setting. Demonstrated experience in the application of Performance Improvement tools including: Lean, PDSA, data collection and analysis, and report writing. Super user in ability to navigate the electronic medical health record Experience in education and coaching. Excellent interpersonal and communication skills. Ability to develop power point presentations using data. Proficiency in Microsoft Office.
