Job Title: Registered Nurse - Case Management Profession: RN Specialty: Case Management Duration: 12 months Shift: Monday - Friday Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years recent RN Case Management and discharge planning experience in a hospital setting License: Must hold a California RN license Certifications: BLS certification (must be through the American Heart Association) Must-Have: Must provide documentation of BLS certification. Must provide documentation of California RN license. Description: Coordinates management of care and ensures optimal utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning. Implements plans and evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through the integration of case management functions. Utilizes utilization review, management, and discharge planning in patient advocacy. Addresses the holistic needs of patients through resource utilization and discharge planning.
