Job Description:
A company is looking for a Care Manager Transition of Care -
RN.
Key Responsibilities
Evaluate member needs through post-discharge assessments and
medication reconciliation
Develop care/service plans and collaborate with healthcare teams to
support member transitions
Facilitate education and resources to promote wellness and improve
overall member care quality
Required Qualifications
Master's degree in Behavioral Health or Social Work, or a degree
from an accredited school of nursing
2 - 4 years of related experience
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
Must reside in the state of Maine
RN Compact License or Multi-State License is a plus
