*Comfort Choice Hospice *is a growing hospice care agency providing quality services to patients in the comfort of their homes. We are seeking a FRIENDLY and DEDICATED *Registered Nurse* *Case Manager (Per Diem Status - Paid per Visit)* to plan, organize, and direct hospice care within the *Coachella Valley. *Applicants should be experienced in nursing with an emphasis on community health education and experience.*Must be able to cover any or all cities between Palm Springs to Coachella/Thermal**Must have 1 year of hospice experience as an RN*_*Registered Nurse Case Manager Responsibilities:*_* Performs initial client assessments.* Oversees admissions, re-certifications, and discharges of our home care clients.* Coordinates the appropriate care plan for clients.* Works collaboratively with other healthcare disciplines.* Administers medications and treatments as prescribed by the physicians in the physician's plan of care.* Work collaboratively with other healthcare disciplines* Has experience with Home Health and Hospice_*Registered Nurse Case Manager Requirements:*_* Licensed as a Registered Nurse/RN* Valid Driver's License and reliable vehicle with current insurance.* Experience with Electronic Medical Records (EMR) software is preferred.* Prior Nursing experience in a TCU, Hospice, or Home Healthcare setting.* Demonstrated ability to work autonomously.* Excellent at planning, organizing, and communicating.* Physical and TB from within 6 months of hire* CPR certification, Driver's License, and Auto InsuranceJob Types: Full-time, Part-timePay: $65.00 - $350.00 per hourBenefits:* Employee discount* Flexible schedule* Mileage reimbursement* Referral programApplication Question(s):* What cities are you looking to take patients in?* Roughly, how many days are you looking to take patients with our company?
