Location: Palm Springs, CA
Salary: $91,000.00 USD Annually - $110,000.00 USD Annually
Description: Our client is currently seeking a RN, Telehealth Case Manager (On-Site) in the Palm Springs area!The Telehealth Case Manager will assist the management team in expanding the telehealth program. This will include, but not be limited to, performing initial contact and remote monitoring of patient and in-home skilled visits. The Telehealth Case Manager is also responsible for developing protocols and pathways for the remote patient monitoring and educating staff and patients on the system. This job will have the following responsibilities:

• Acts as a liaison between acute and post-acute care by promoting the utilization of telehealth on patients discharging from the acute setting meeting criteria. -
• Reviews referrals to determine if the patient may benefit from telehealth. -
• Works with the telehealth team on initiating the remote monitoring equipment. -
• Makes the initial skilled nursing visit to assess patient knowledge and helps to determine the correct pathways for the remote monitoring system. -
• Monitors the dashboards and available for skilled intervention if required. -
• Makes in home visits as ordered and PRN based on patient condition. -
• All other duties assigned. Qualifications & Requirements:
  • Current California RN License required.
  • Minimum 6 months working in telehealth or remote patient monitoring experience required. Minimum one year of Home Health or Triage experience preferred. -
  • Working knowledge of healthcare based computer systems. -
  • Ability to manage time and stay organized. -
  • Excellent assessment and nursing skills. -
  • Good communication skills, detail oriented, critical thinking and problem solving skills. -
  • Excellent customer service and telephone etiquette skills. -
  • BLS required within 60 days of hire. -
  • Driving required. Current California Driving License and proof of insurance. -
