Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical health needs.
Key Responsibilities
Evaluate member needs and recommend care plans to address barriers
to accessing appropriate care
Coordinate with providers and community resources to ensure members
receive person-centered care
Monitor member progress and update care plans as necessary to meet
individual goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - Compact State Licensure required
Experience in physical health and chronic disease management
Bilingual in Spanish or Creole preferred
